Medical Documentation for VA Claims

Our veteran-owned practice serves veterans nationwide. Evaluations and opinions are completed by a Physician.

We review medical records, relevant history, and clinical evidence for VA claims and appeals. Consultation or examination needs depend on the case. Conclusions reflect the available evidence; no favorable opinion or claim outcome is guaranteed.

  • Nexus letters explain whether a condition is related to military service or to an already service-connected disability. We address:

    • Direct service connection

    • Secondary service connection (causation)

    • Aggravation of a preexisting condition by military service

    • Aggravation of a condition by a service-connected disability

    Each opinion includes medical reasoning based on the evidence in the individual case.

  • Independent review of medical records and disputed medical questions for initial claims, supplemental claims, and appeals. Reports explain findings and reasoning, including clarification or rebuttal of prior medical opinions when appropriate.

  • Disability Benefits Questionnaires (DBQs) document diagnoses, symptoms, severity, and functional limitations. Evaluation and examination requirements vary by condition and the available records.

  • Medical assessments of disability severity, functional impact, and changes over time for increased-rating claims. TDIU opinions address how service-connected conditions affect the ability to secure and follow substantially gainful employment, based on the individual evidence.

  • Medical opinions addressing toxic exposure risk activities (TERA), environmental exposures, and related medical questions. Reviews may include presumptive conditions and Gulf War illness questions when medically appropriate; a nexus opinion is not required for every presumptive claim.

  • Cause-of-death and contributory-cause medical opinions for survivor / Dependency and Indemnity Compensation (DIC) claims. Each review addresses the relevant medical questions and evidence.

    VA tort claims are not offered.

    • Aid and Attendance, housebound status, and Special Monthly Compensation (SMC): medical findings about daily assistance needs, functional limitations, and related benefit questions.

    • Retrospective opinions: medical assessment of past severity, onset, and progression using the available records.

    • Competency opinions: assessment of the medical ability to manage VA benefits.

    These reports address medical evidence; VA determines benefit eligibility and legal requirements.
